Patient Access

In order to register for online access (we use an external system called "Patient Access") in which you can order repeat medications and make routine appointments please complete the attached form and email to the below address. We will set you up and reply to your email provided with your log in details and instructions on how to set this up.

Please note as this service is not run by the practice we cannot help with account unlocks - please refer to the help/assistance icon on the patient access page. 

Please note patients from the age of 12 years, who are judged by the GP to be able to make decisions on their own must agree if the practice is to give out medical information to their parent/guardian. Therefore if the contact details given on the form are not that of the patient and contain details of a parent or guardian we will require written consent from the patient to process this request.
