Registration Process
In order to register you must complete the GPR form and the appropriate Blackfriars questionnaire for each patient
Completed registration forms should be emailed to Fife.f21825blackfriars@nhs.scot
Once we have received the forms we will process the registration within 2 working days. Please note you will not receive confirmation of your registration. You cannot get an appointment with the practice until you are registered. All UK applicants would need to contact their previous GP as they will remain registered with them until we accept you.
We also require photo I.D for each patient and proof of address
Please note we only accept registrations from patient's living within our catchment area (map below). If you move out of the catchment area then you will need to register with another GP practice.
